NorCal Region Good Health WINs Mini-Grants

The NorCal Region of National Council of Negro Women, Inc. (NCNW) mini-grant program engages NorCal Region Sections to partner and coordinate with jurisdictions and participating pharmacies in the area to enhance efforts of the CDC Bridge Access program to reach uninsured and underinsured adults and improve vaccine uptake and access and to partner with jurisdictions and pharmacies to reach uninsured and underinsuredadults in low vaccination coverage areas, and in communities that have been historically underserved in the health system.

Eligible organizations can apply to receive mini-grants between $500-$1,000. Due to limited funds, not all applications submitted will receive funding. See below for eligibility requirements.

Goal:

The mini-grants will support efforts to partner and coordinate with jurisdictions and participating pharmacies in the area to enhance efforts of the Bridge Access Program to reach uninsured and underinsured adults and improve vaccine uptake and access and to partner with jurisdictions and pharmacies to reach uninsured adults in low vaccination coverage areas, and in communities that have been historically underserved in the health system.

Sections may only submit one application at a time for a maximum of $1,000. Once the first project is completed and the final report for that project is submitted and approved, the Section will be eligible to submit a new application.

All mini-grant projects must be completed by Sunday, December 15, 2024

Submit a monthly Budget to Actual Report to Cheryl Johnson by the 20th of the month.

Submit monthly electronic reports, Smartsheet to NCNW National no later than the 20th calendar day of each month. Information should include:

Funding

Mini-grant funding will be distributed in two installments. The first installment, 75% of the grant, will be distributed after award is announced. The final 25% of the grant will be awarded upon completion of the project and submission of the final report. Payments will be mailed to the Section mailbox.

NorCal WILL delay or withhold funds from Sections if there is a failure to: complete any reporting requirement; or establish or maintain a Section Project Team; or attend CoP sessions; or submit the monthly or final report.

Repeated failure to fulfill program responsibilities will result in dismissal from the NorCal Mini-Grant Program and repayment of allocated funds. Notice of dismissal shall be made to the Section’s President and Project Lead.

A representative from NorCal’s Good Health WINs Team will contact the recipient via email and/or phone call.

There will also be a mandatory orientation for all awardees and their teams on Thursday, April 25, 2024, via Zoom.

  • Attend bi-monthly Good Health WINs Community of Practice meetings, on the 1st and 3rd Thursday.
  • Attend monthly regional Good Health WINs meeting, to be announced.
  • Submit monthly Smartsheet project reports on the 20th of each month.
  • Submit monthly Budget to Actual project reports to the Project Manager on the 20th of each month.
